Inspection Notice – 3M™ DBI-SALA® ExoFit NEX™ Harnesses – Immediate action required

3M Fall Protection 260 Export Boulevard
Mississauga (Ontario) L5S 1Y9
1-833-998-2243
Inspection Notice – 3M™ DBI-SALA® ExoFit NEX™ Harnesses

IMMEDIATE ACTION REQUIRED
3M Fall Protection has learned of the possibility of a manufacturing defect in a dorsal d-ring of ExoFit NEX™ harnesses manufactured between January 2016 and December 2018. Although there have been no reported incidents involving this condition, a dorsal d-ring with this defect will not support the load in a fall arrest event which could result in serious injury or death. Harnesses manufactured only within this date range require immediate inspection for lot number 09P1 stamped into a dorsal d-ring. We believe that only one harness was manufactured with a defective D-ring, but we urge inspection of all potentially affected harnesses out of an abundance of caution in the interests of worker safety.

End Users: Upon receipt of this Notice, immediately inspect your harness following the steps below:
Step 1: Locate the label pack on the harness to identify the manufacturing date. If the harness has a manufacturing date of 16/01 (2016, January) through the end of 18/12 (2018, December), continue to step 2. If the harness is not in this range, the unit is not impacted by this notice. If the harness is within this date range, continue to step 2.
Step 2: Locate the D-ring on the back of the harness to inspect for a stamped lot date of 09P1. If you find a D-ring with code 09P1 and the harness has a manufactured date within the affected date range, take the harness out of service immediately. If the D-ring is not stamped with code 09P1, you may continue using your harness.
Please note that both the manufactured date range (2016, January through 2018, December) on the harness label AND the lot number code 09P1 stamped on the D-ring must be present on the same harness for the harness to be considered suspect and removed from service. All other harness/d-ring combinations are acceptable for use.

End-users: If you find an affected harness, take the unit out of service immediately. You can contact us at 3M Customer Service at 1-833-998-2243 or at 3MCAFPServiceAction@mmm.com to return your harness and a replacement harness will be provided free of charge.

3M remains committed to providing quality products and services to our customers. We apologize for any inconvenience that this situation may cause you or your customers. We appreciate your continued support of 3M Fall Protection products and services.

The New Astro Bod Fast

ULTRA COMFORTABLE ROPE ACCESS HARNESS

With an integrated CROLL L ventral rope clamp, the ASTRO BOD FAST harness is designed for greater efficiency and comfort during rope ascents.

Wide, semi-rigid waistbelt and leg loops give excellent support. They are contoured and lined with breathable foam padding for comfort when suspended.

Metallic side attachment points allow easy connection of equipment and can be folded down to prevent them snagging accidentally when not in use.

The international version has a fall indicator: a red strap appears on the dorsal attachment point after a fall to indicate that the harness should be retired.
Foam shoulder straps are widely spaced to reduce neck chafing. When the waistbelt is loaded, they help distribute the load over the shoulders

Specifications

  • Ventral attachment point: connection of a descender, a positioning lanyard for use in single mode, a progression lanyard, and a seat
  • Lateral attachment points: attachment for a positioning lanyard for use in double mode
  • Sternal attachment point: fall arrest system attachment

  • Rear attachment point on the waistbelt: restraint lanyard attachment

PRODUCT RECALL NOTICE – CMC Flash™ Hook and FlashLite

cmcpro.com
6740 Cortona Dr.
Goleta, CA 93117
800-235-5741
805-562-9120
Product Recall Notice: CMC Flash™ Hook and FlashLite™ Hook (P/N
300980 & 300981)
The issue:
During product validation testing of a newly developed escape hook, CMC discovered
the test fixture used to certify the existing Flash™ Hooks and FlashLite™ Hooks
unintentionally influenced the NFPA 1983 strength results when tested at Underwriters
Laboratory (UL). Ongoing testing indicates that in some applications Flash™ Hooks
and FlashLite™ Hooks may be lower in strength than the Minimum Breaking Strength
(MBS) marked on the products.
CMC has no knowledge of failures or injuries:
The hook strength issue was found during product testing and we have no knowledge
of failures or injuries with the many thousands of Flash™ Hooks and FlashLite™ Hooks
in use over the past decade. CMC stands behind our product quality and intends to offer
replacements for all hooks currently in service with an updated design providing a higher
margin of strength and safety.
What CMC will do:
CMC has completed and validated the new design and is working closely with UL to
implement a new test fixture and certify this replacement hook as soon as possible. CMC
will replace all Flash™ Hooks and FlashLite™ Hooks, with the redesigned, stronger hook
at CMC’s expense.
In the Meantime:
CMC considers current Flash™ Hooks and FlashLite™ Hooks safe to use in the interim
when applied in accordance with the user instructions (attached). For training evolutions,
always use a competent belay.
What you should do to participate in the recall program:
Details of the replacement program will be communicated as soon as possible. Please
register at the address below to assure we can contact you with updates about when
replacements will be available (estimated in March, 2019).
Our top priority is user safety. We will do our best to minimize inconvenience while
conducting this recall program. Thank you for trusting CMC products for your life safety
needs.
